Children's Institute provides a variety of professional assessment measures for use with young children. Used separately or together, these measures help to establish a clear picture of a child's problems for the purpose of planning intervention goals, and to determine the attainment of relevant program outcomes.

AML-R Behavior Rating Scale (AML-R) is a 12-item, quick-screening tool completed by teachers to aid in identifying children experiencing school adjustment problems.

Associate-Child Rating Scale (A-CRS) is a 20-item, child-rating instrument completed by a service provider. It assesses both a child's relative strengths and weaknesses compared to other program-referred children.

Background Information Form (BIF) provides a systematic way to gather child information so as to better understand the child and to develop program goals.

Child Log (CL) provides a systematic record of child contacts. This information is often used to indicate the range of helping services provided to the child, which is often required by funding sources.

Child Rating Scale (CRS) is a 24-item, self-rating scale for children to complete. It can be used as a screening tool to identify children experiencing school adjustment problems, to make referrals to special services programs, or to assist in determining individual or program service strategies.

Parent Appraisal of Children's Experiences (PACE) provides school personnel, districts and the community at large with a picture of the health, family situations and school readiness skills of students entering kindergarten.

Parent-Child Rating Scale (P-CRS) is a 39-item measure that assesses children's behavioral, social, and emotional functioning from a parent's perspective. It has excellent psychometric properties.

Professional Summary Report (PSR) summarizes a child's program experience and is typically used for Primary Project.

Substance Abuse Measure (SAM) is a comprehensive, child-completed survey that assesses information regarding a variety of illicit substances.

Screening and Evaluation Measures and Forms Guidelines provides a rationale, background, and description of each instrument, as well as administration and scoring instructions.

Teacher-Child Rating Scale 2.1 (T-CRS 2.1) consists of 32 items assessing different aspects of a child's social and emotional adjustment, enabling teachers to convey information about a student's behavior that's accurate and discriminate.
