AML Behavior Rating Scale (AML-R)
Teacher completed screening measure of child’s behavior.
Associate-Child Rating Scale (A-CRS)
Used by child associates who work in Primary Project or in a one-to-one setting with a child to gather data on child’s social and emotional health.
Attendance Records
Developed for preschool and education programs to record attendance.
Background Information Form (BIF)
Completed by teacher or primary project child associate to gather education, student and social economic status, and parent information.
Child Rating Scale (CRS)
Completed by children in grades 2-6 to assess social and emotional health.
Early Childhood Parent Survey
Parent completed assessment of early childhood program quality and parental satisfaction.
Future Expectations
Assesses parents’ expectations of their children. Completed by parents and used in resiliency research.
Parent Appraisal of Children’s Experiences (PACE)
Completed by a parent of a child who is entering kindergarten assessing the following: family background, general health of child, motor, speech and language, social emotional development, life experiences and school readiness.
Pre-kindergarten PACE
Completed by a parent of a child who is entering pre- kindergarten assessing the following: family background, general health of child, motor, speech and language, social emotional development, life experiences, school readiness; discipline, limit setting, routines, sensory motor, play scale, and parenting scales.
Parent Questionnaire 2.0
Available in pre and post versions, parents complete this questionnaire to assess their needs and expectations of the pre-kindergarten classroom environment.
Parent-Child Rating Scale (P-CRS)
Parent completed for children in grades pre-k through 6th. Assesses resiliency in children.
Perceived Self-Efficacy Scale (PSE)
Available for grades 2-3 and grades 4 and higher. Completed by child to assess self-worth.
Professional Summary Report (PSR)
Completed at year-end by professionals who have children enrolled in Primary Project.
Social Support Questionnaire
Assesses parents’ perceptions of their children’s social support networks. Completed by parents.
Stage of Change Scale for Early Education and Care 2.0
Designed to measure caregivers’ readiness to change their child care practices to benefit children. Completed by early childhood educators, child care center administrators, and professionals who mentor early childhood educators.
Teacher-Child Rating Scale (T-CRS 2.1)
Completed by teachers of grades pre-k to 12, this assessment rates children’s social and emotional development in areas including initiative, independence, and coping ability.
What I Think About Myself
Completed by children in grades 6-8, this assessment offers information on risk and protective factors.
