The Child Log (CL) provides a systematic record of child contacts. This information is often used to indicate the range of helping services provided. Such logs may be required by funding sources. The CL can assist in documenting the distribution of resources.
The CL collects:
Number of individual/group sessions (plus space for notes)
Service provider ID number
Average session length
Typical number of sessions per week
Administration: Service provider
Scannable paper forms can be returned to Children’s Institute to quickly score and summarize the data you’ve collected. Profiles, percentiles, and reports showing total strengths, needs, and trends are provided where applicable. Pre/post evaluations and data files are also available.
