The Stage of Change Scale is a simple rating form that consists of 6 items assessing parent/caregiver readiness to change their child care practices or other personal goals. The scale is based on the Transtheoretical Model of Change, a framework developed in the field of behavioral health and endorsed by the National Institutes of Health.
